How to Order Testing

Ordering testing from Ambry has never been easier! We offer two different methods to send your patient’s sample to Ambry. Follow these quick steps so we can help you find the answer.

Method 1: Using AmbryPort 2.0 (AP2), our secure online portal

AP2 allows you to submit orders, review order status, submit insurance preverification requests, complete an online family history, and receive result reports securely. Follow these steps to submit your order to AP2:

  1. Order a Sample Submission Kit at no cost by visiting here
  2. Create an account and log into AP2
  3. Once you have logged into your AP2 account, complete the appropriate test requisition form (TRF) and patient family history information (for Cancer orders only). Submit your order online and send in your sample.
  4. Include the printed TRF as well as the insurance information (when applicable) within the Sample Submission Kit.
  5. Ship Sample Submission Kit to Ambry using the pre-paid FedEx or DHL (optional for international orders) envelope.
  6. Retrieve your secured result reports electronically

Method 2: Using a paper Test Requisition Form (TRF)

  1. Order a Sample Submission Kit at no cost by visiting here
  2. Complete the appropriate Test Requisition Form, which can be found in your Sample Submission Kit or download it here.
  3. For Cancer orders only: complete and print our family history questionnaire here.
  4. Then add into the Sample Submission Kit and also include the insurance information (when applicable). 
  5. Ship Sample Submission Kit to Ambry using the pre-paid FedEx or DHL (optional for international orders) envelope.  
  6. Receive your result report securely by AP2 (if you have an account setup), fax or mail. 

 

You can always refer to the Minimum Information Required for Test Orders form to ensure all required information on the TRF is included before submitting your genetic test order.
